1.  Acceptance & Updates

We may revise Terms as laws change. If you keep using the site after an update, you accept the updated version. A banner will highlight material changes for 30 days.

2.  Educational Content; Not Emergency Care

Blogs, guides, and videos are educational. They do not replace consultation with a licensed professional. For emergencies, please dial 911 or visit the nearest ER.

3.  Licensure Boundaries

Our board-certified integrative physicians currently hold active medical licenses in Illinois, Maryland, North Carolina, and California. We deliver in person care only to patients physically located in Chicago, Illinois. We deliver telemedicine care to residents in MD, NC, and CA. We do not treat medical emergencies.

4.  Appointment Policies

Booking & Payment
Fees appear in the scheduler and are charged immediately to hold your slot.

Rescheduling or Canceling
There is a $150 cancellation fee if a cancellation is less than 24hrs prior to the start of your appointment. This is due to the demand for appointments and our long wait list.

No-Show
No show, no call, costs 100% of consultation fee.

5.  Fees, Insurance & Refunds

We are fee-for-service and do not bill insurance directly. Major credit cards, HSA, and FSA are accepted. Refunds are issued only when we cancel and cannot reschedule, or when a prepaid service is never delivered because of our error. You receive a superbill with CPT and ICD-10 codes for possible out-of-network reimbursement.

6.  Intellectual Property

All site text, graphics, videos, and downloadable resources belong to My Native Doctor. You may print or download materials for personal use but may not republish or sell them without written permission.

7.  User Conduct

You agree not to upload malware, scrape data, impersonate others, or harass staff or patients. Violation may lead to account termination and legal action.

8.  Third-Party Links

External links are provided for convenience. We do not control or endorse third-party content and are not responsible for their security or privacy practices.

9.  Limitation of Liability

Our liability for any claim related to site use or services is limited to the amount you paid for that service. We are not responsible for indirect, incidental, or consequential damages.

10. Indemnification

You agree to indemnify and hold harmless My Native Doctor, its owners, and employees from any claim arising out of misuse of the site or violation of these Terms.

11. Governing Law & Venue

These Terms are governed by Illinois law. Any dispute shall be resolved in the state or federal courts located in Cook County, Illinois.
