GLP-1 Weight Loss Medications During Menopause: What Women Should Know About Bone Health
For many women, menopause brings changes that can feel frustrating and difficult to explain. Body composition may shift, abdominal fat may increase, and losing weight can become more challenging even when eating and exercise habits have not changed dramatically.
As GLP-1 medications have become increasingly popular for weight management, many women are asking an important question:
If I use a GLP-1 medication to lose weight during or after menopause, what does that mean for my bones and muscles?
It is a thoughtful question—especially because menopause itself is a time when bone loss accelerates.
The answer, however, is more nuanced than simply saying that GLP-1 medications cause bone loss. Current research does not establish that GLP-1 medications inherently weaken bones. Instead, there are several overlapping factors to consider: menopause-related changes in estrogen, the effects of weight loss on bone and lean tissue, nutrition, physical activity, and an individual's underlying fracture risk. Research specifically examining GLP-1 medications and bone health is still evolving.

Why Menopause Matters for Bone Health
Bone is living tissue. Throughout our lives, old bone is broken down and replaced with new bone in a continuous process called bone remodeling.
Estrogen plays an important role in maintaining this balance.
As estrogen levels decline during the menopausal transition, bone remodeling can become more rapid, with bone breakdown outpacing bone formation. This makes the menopausal transition an important period for bone health.
That doesn't mean every woman will develop osteoporosis after menopause. Bone health is influenced by many factors, including genetics, age, body weight, physical activity, nutrition, smoking, alcohol use, medications, and other medical conditions.
But menopause is an important reason to start thinking about bone health proactively.
Where Does Weight Loss Fit In?
Weight loss can have many health benefits, particularly for people living with overweight or obesity. Improving metabolic health, blood pressure, blood sugar, sleep apnea, mobility, and cardiovascular risk may all be meaningful goals.
At the same time, substantial weight loss can affect more than body fat.
When the body loses weight, some lean tissue can be lost along with fat. Bone is also sensitive to changes in mechanical loading and energy balance.
This is one reason that the goal of medically supervised weight loss should not simply be "lose as much weight as possible."
A better question is:
How can we improve metabolic health while preserving as much muscle and bone health as possible?
That question becomes particularly important during midlife and menopause.
Do GLP-1 Medications Cause Bone Loss?
This is where the conversation requires some nuance.
GLP-1 medications—including medications containing semaglutide, tirzepatide, and other GLP-1 receptor agonists—can produce substantial weight loss. Because weight loss itself can influence bone density and lean mass, researchers are actively studying what happens to the skeleton and muscles during GLP-1 treatment.
So far, the evidence does not support a simple conclusion that GLP-1 medications directly cause osteoporosis or increase fracture risk.
A 2025 systematic review and meta-analysis of randomized trials in people with type 2 diabetes found that GLP-1 receptor agonists were not associated with an increased fracture risk and were actually associated with improvements in bone mineral density at several sites. The researchers emphasized, however, that limitations in the available studies mean more high-quality, longer-term research is needed.
More recent research has continued to produce mixed and evolving findings. A 2026 analysis presented by the Endocrine Society, for example, found that semaglutide was associated with a lower fracture risk compared with other anti-obesity medications in people with type 2 diabetes, although the researchers emphasized the need for prospective studies to confirm the finding.
In other words, the medication itself should not automatically be viewed as a threat to bone health.
The bigger issue may be how weight loss is achieved and what happens to the rest of the body during that process.
Muscle Matters, Too
Bone health and muscle health are closely connected.
Muscles provide mechanical stimulation to bones, while strength and balance help reduce the likelihood of falls—the events that can ultimately lead to fractures.
GLP-1-associated weight loss can include some loss of lean mass. This isn't necessarily unique to GLP-1 medications; lean tissue can be lost during weight loss more generally.
A 2026 systematic review of randomized trials found that GLP-1 receptor agonists reduced absolute lean mass while increasing lean mass as a proportion of total body weight because fat mass generally decreased more substantially. The researchers concluded that nutrition and physical activity should accompany medication treatment to help preserve or improve muscle.
For women approaching or going through menopause, this deserves particular attention.
The goal shouldn't simply be to become lighter.
The goal is to become healthier while maintaining strength, function, and physical resilience.
Strength Training Is Part of the Prescription
If you're using a GLP-1 medication for weight loss, resistance exercise can be an important part of protecting lean tissue.
Strength training provides the muscles with a reason to remain strong during weight loss. Weight-bearing and resistance exercise can also benefit bone density.
Research in postmenopausal women supports this approach. A 2024 randomized controlled trial found that a 12-month resistance and impact exercise program benefited hip bone structure in postmenopausal women with low bone mass.
A 2024 systematic review and meta-analysis likewise found beneficial effects of exercise on bone density and physical performance in postmenopausal women.
This doesn't mean every woman needs an intense gym program.
Depending on fitness level and medical history, strength-focused movement might include:
Resistance bands
Bodyweight exercises
Free weights
Weight machines
Squats or sit-to-stand exercises
Stair climbing
Walking and other weight-bearing activities
The right program should be individualized, particularly for women with osteoporosis, joint problems, previous fractures, or other medical conditions.
Don't Forget Protein
Another consideration during GLP-1-assisted weight loss is nutrition.
Because these medications can reduce appetite, some people may unintentionally eat much less than they need. Over time, inadequate nutritional intake can make it more difficult to preserve lean tissue and meet essential nutrient needs.
Protein-rich foods can be particularly important for supporting muscle during weight loss.
Rather than focusing only on calories, an integrative approach considers the quality and nutritional adequacy of the diet.
Depending on individual needs, this may mean emphasizing:
Adequate protein
Fruits and vegetables
Fiber-rich foods
Calcium-containing foods
Vitamin D sources
Healthy fats
Adequate hydration
A wide variety of nutrient-dense foods
The specific nutritional requirements of someone using a GLP-1 medication will vary based on their health, medications, activity level, age, and weight-loss goals.
Should You Get a Bone Density Scan?
Not every woman beginning a GLP-1 medication needs a bone-density scan simply because she is taking the medication.
Bone-density screening is based on overall fracture risk.
The 2025 U.S. Preventive Services Task Force recommends osteoporosis screening for women 65 and older. For postmenopausal women younger than 65, screening is recommended when a woman has risk factors that place her at increased fracture risk after clinical risk assessment.
Risk factors can include things such as low body weight, a parental history of hip fracture, smoking, excess alcohol use, certain medications, and other medical conditions.
For some women, particularly those entering menopause with additional risk factors, discussing bone health before or during significant weight loss may be worthwhile.
Your clinician can help determine whether a DXA scan or additional assessment makes sense for you.
What About Calcium and Vitamin D?
Bone health isn't determined by one nutrient.
Calcium and vitamin D are important, but simply taking supplements isn't a substitute for an overall bone-health strategy.
Nutrition, physical activity, hormonal status, body composition, medications, smoking, alcohol intake, and other health conditions all matter.
It's also worth avoiding the assumption that everyone needs the same dose of calcium or vitamin D supplements. Individual needs should be assessed based on diet, age, medical history, medications, and laboratory findings when appropriate.
The goal is adequate nutrition—not automatically taking more supplements.
A More Complete Way to Think About GLP-1 Weight Loss
The conversation around GLP-1 medications can sometimes become overly polarized.
Some people view them as a miracle solution.
Others view them as inherently dangerous.
Neither perspective captures the full picture.
For people who meet medical criteria for pharmacologic weight management, GLP-1 medications can be valuable tools. The World Health Organization's 2025 guideline recommends that GLP-1 therapies for obesity be used as part of comprehensive care that includes healthy nutrition, physical activity, and ongoing support.
At the same time, medication is only one part of the picture.
For a woman in menopause, a comprehensive plan might consider:
Metabolic health
Blood sugar, blood pressure, cholesterol, and cardiovascular risk.
Hormonal health
The symptoms and health considerations associated with perimenopause and menopause.
Muscle health
Strength, physical activity, protein intake, and preservation of lean mass.
Bone health
Fracture risk, bone density, nutrition, and weight-bearing activity.
Digestive health
Appetite changes, nausea, constipation, reflux, and overall nutritional tolerance.
Sleep and stress
Both of which can influence appetite, metabolism, recovery, and quality of life.
This is the kind of whole-person perspective that can make weight management more sustainable.
Weight Loss Shouldn't Come at the Expense of Strength
For women going through menopause, the scale is only one measure of health.
A successful health journey isn't necessarily the one that produces the fastest or largest drop in body weight.
It may be the one that improves metabolic health while helping you remain strong, active, nourished, and resilient.
If a GLP-1 medication is part of your treatment plan, that doesn't mean bone and muscle health have to be overlooked. In fact, they should be part of the conversation from the beginning.
